Job Responsibilities
- Supervise and coordinate clinical services for patients in their homes.
- Provide and direct nursing care to patients as prescribed by the physician.
- Ensure compliance with applicable laws, regulations, and agency policies.
- Receive referrals and ensure appropriate clinician assignments.
- Evaluate patient orders and plot start of care visits.
- Coordinate determination of patient home health benefits and ongoing insurance approvals.
- Assess patient needs continually and provide individualized care.
- Review assessments and plans of care daily and consult clinicians with recommendations.
