Job Responsibilities
- Conduct triage and symptom management via high-volume call handling.
- Coordinate care and referrals across clinical and community settings.
- Provide direct patient care and education during treatments and assessments.
- Participate in geriatric syndrome evaluations and follow-up care.
- Conduct comprehensive assessments with patients and families to develop individualized care plans.
- Monitor symptoms and treatment responses, ensuring proactive follow-up.
- Maintain accurate documentation and adhere to program guidelines.
- Participate in ongoing training and staff meetings.
- Ensure compliance with federal, state, and institutional standards.
- Perform other duties as assigned.
