Conduct triage and symptom management via high-volume call handling.
Coordinate care and referrals across clinical and community settings.
Provide direct patient care and education during treatments and assessments.
Participate in geriatric syndrome evaluations and follow-up care.
Conduct comprehensive assessments with patients and families to develop individualized care plans.
Monitor symptoms and treatment responses, ensuring proactive follow-up.
Maintain accurate documentation and adhere to program guidelines.
Participate in ongoing training and staff meetings.
Ensure compliance with federal, state, and institutional standards.
Perform other duties as assigned.
