Job Responsibilities
- Advocate for referred patients in the pre-admission process by collaborating with clinical teams to assess patient needs.
- Serve as a resource for healthcare professionals through fostering relationships within medical facilities.
- Coordinate care plans with physicians and families using clinical expertise and problem-solving skills.
- Facilitate patient transfers and address barriers to accessing specialized post-ICU care.
- Work with physicians, discharge planners, and patient's families to plan a continuum of care after an ICU or post-surgical stay.
- Perform clinical assessments of referred patients and counsel with patient's families to assist them in care decisions.
- Partner with hospital case managers and physicians to create smooth pre-admissions and transfer processes.
Required Skills
Core Skills
- Registered Nurse (RN) or Respiratory Therapist (RT) License
- Ability to complete clinical assessments of medically complex patients
Preferred Skills
- Demonstrated success in patient advocacy
- Superior communication and relationship-building skills
- Prior healthcare marketing or business development experience
