Job Responsibilities
- Teach and uphold Northern Illinois Hospice's standards with a focus on 'I Am a Teacher', perfect documentation, perfect visits, perfect communication, exceptional customer service, teamwork, and professionalism.
- Act as a clinical practice expert/resource to staff.
- Manage the RNs, LPNs, CNAs, Social Workers, Hospice Chaplain/Spiritual Counselors, Aides, and complementary therapists who care for patients.
- Manage the team's day-to-day operations, including scheduling, performance, budget, utilization, and quality assessment/improvement processes and methodologies.
- Develop and coach staff to create an excellent team.
- Provide direct and daily supervision to the team.
- Perform initial, annual, and periodic field competency visits focusing on infection control, quality, and safety.
- Adhere to and teach Joint Commission accreditation standards of practice.
- Participate in audits to ensure regulatory and agency compliance.
- Manage team's direct care and documentation from admission to discharge.
- Coordinate team's IDG meetings, demonstrating excellent communication and teaching skills.
- Proactively manage to prevent service failure with patient/family/facilities.
- Monitor scheduling to ensure all team's on-call hours are covered appropriately.
- Responsible for timely annual and ongoing performance evaluations of all team members.
- Participate in clinical administration on-call as assigned.
- Serve as backup for clinical on-call rotation as needed.
- Collaborate with the Director of Clinical Services, Medical Director, and clinical managers regarding patient care.
- Work with the Director of Quality and Compliance to identify and address QAPI and education opportunities.
- Actively participate with the Management team and quality assessment performance improvement teams and activities.

Requirements
Experience: At least two years in management or supervision required.
Education: Associate degree in nursing required, Bachelor of Science in Nursing (BSN) preferred.
License & Certification:
- Professional Registered Nurse License issued by the Illinois Board of Nursing
