Job Responsibilities
- Navigate patients through the program for treatment in a timely manner.
- Facilitate rapid scheduling and follow-up tracking of patients to ensure completion of their recommended treatment plan.
- Identify and problem-solve any service gaps identified.
- Promote the Navigator role and treatment awareness to the community through presentations and participation in related organizations.
- Staff multidisciplinary conference and coordinate loop closure with referring providers.
- Collect quality data on patients.
