Implements assessment and care based on the developmental stages of the patient.
Involves family/guardian in the plan of care and conducts patient and family education as appropriate.
Provides direct social work service and grief counseling to patients and their families.
Identifies strengths and resources available to the patient and family; documents recommended actions, interventions, or referrals.
Collaborates with multidisciplinary teams to develop and implement discharge planning for patients upon admission.
In conjunction with the C3 RN, initiates the development of discharge plans for complex patients.
Collaborates with healthcare team members to determine goals for length of stay management and discharge management activities.
Identifies high risk, vulnerable populations and intervenes to coordinate care for these patients.
Facilitates community linkage and referral on behalf of patients to assist in arranging medical follow-up.

Experience: Two years of experience in an acute care setting required.

Education: Bachelor’s of Science in Nursing required.

License & Certification:

Current licensure to practice nursing in the State of Illinois
Case management certification required or obtained within one year of eligibility
