Position: Community Alternative Program – Disabled Adults
Job Type: Exempt (salary)
Pay: $75,292.00 - $98,820.75/yr
Shift: Monday – Friday 8:00am – 5:00pm

Job Responsibilities
- Coordinate care needs across the healthcare continuum to ensure patients receive efficient and effective care.
- Serve as a liaison between the clinic and the patient/family for education, counseling, and communication.
- Provide ongoing support to clinical staff and external constituencies to ensure operational success.
- Respond in cases of urgent and emergent public health events requiring interventions.
- Administer immunizations, mass medication distribution, and provision of health services in shelter environments during public health events.

Requirements
Experience: Minimum of two years of nursing case management experience.
Education: Bachelor’s degree or higher in Nursing from an accredited college.
License & Certification:
- Current license to practice as a Registered Nurse (RN) in NC or another Nurse Licensure Compact State.
- Certification in Basic Life Support for Healthcare Providers (CPR & AED) by the American Heart Association.
