Job Responsibilities
- Provides direction and guidance to the hospice program under the supervision of the Medical Director.
- Liaises with MD, Attending Physician, other Nurse Practitioners, staff, and volunteers to ensure quality care for patients and families.
- Acts as a contact between the hospice program and community physicians and others engaged in healthcare services to develop and expand the patient base for Hospice.
- Provides direct medical care as needed to patients in their homes, nursing homes, and hospitals.

- Experience in Hospice or Home Care
- CHPN certification
Requirements
Experience: Nursing experience required; experience in Hospice or Home Care preferred.
Education: Graduate of a Master’s Degree Nursing Program
License & Certification:
- Current ARNP license for assigned state
- Current driver's license for the state of residency
