Job Responsibilities
- Provide holistic, client-centered care to specialized populations with barriers to care, including pediatrics, refugees, newcomers to Canada, trans care, & palliative care.
- Conduct assessments, diagnoses, planning, implementation, and evaluations in-person and virtually to your own roster of patients.
- Perform home visits for clients unable to visit the clinic.
- Collaborate with interdisciplinary teams to deliver high-quality care.
- Engage in quality improvement, research initiatives, and student mentorship.
- Utilize Electronic Medical Records (EMR) efficiently for documentation.
Required Skills
Core Skills
- Degree in Nursing from a recognized institution
- Current extended class registration with the College of Nurses of Ontario
- Minimum of 3 years of clinical experience, preferably in a community setting
- Excellent communication and interpersonal skills
- Proficiency in EMR and virtual care tools
- Commitment to continuous learning and professional growth
- Ability to work flexible hours and participate in on-call schedules
- Valid Ontario Driver’s License and access to a reliable vehicle
Preferred Skills
- Passion for working with socially and medically complex patients
Requirements
Experience: Minimum of 3 years of clinical experience, preferably in a community setting
Education: Degree in Nursing from a recognized institution
License & Certification:
- Current extended class registration with the College of Nurses of Ontario
Schedule:
