Develop and facilitate complex care management activities for members with physical needs.
Evaluate member needs and barriers to accessing care, focusing on member-identified priorities.
Develop ongoing care/service plans and collaborate with providers to address unmet needs.
Identify problems/barriers to care and provide appropriate management interventions.
Coordinate between members, families, caregivers, and care provider teams to ensure person-centered care.
Provide follow-up and monitoring of member status and revise care plans as necessary.
Support members and care managers with local resource information for various services.
Facilitate care management and collaborate with providers to ensure timely access to needed services.
Perform outreach, including telephonic and home visits, to assess member needs.
Document and maintain member information and care management activities in compliance with regulations.
Educate members and their families on healthcare processes and options.
Provide feedback to leadership on care improvement opportunities.

Experience: Minimum of two (2) years of pediatric experience required.

Education: Degree from an Accredited School of Nursing or Bachelor's degree in Nursing.
