Job Responsibilities
- Conduct comprehensive OASIS Start of Care, Recert, Resumption, and Discharge assessments
- Perform wound care, including complex wound management
- Administer IV therapy and medications
- Manage chronic diseases and educate patients and families
- Collaborate with PT/OT/ST and the interdisciplinary team
- Adhere to current CMS, Medicare, and PA DOH standards
Required Skills
Core Skills
- RN license in PA
- Knowledge of OASIS and skilled nursing criteria under CMS guidelines
- Excellent time management
Preferred Skills
- Prior home health experience
- Reliable transportation
Requirements
