Job Responsibilities
- Provide therapeutic patient and family centered care.
- Systematically and continuously collect and assess data related to patient health status.
- Develop, execute, and evaluate the plan of care.
- Utilize evidence to drive clinical decisions and practice related to individual and population health.
- Establish goals and strategies for meeting care coordination needs across settings.
- Consider factors related to safety, effectiveness, and fiscal accountability in planning and delivering care.
- Delegate and supervise care in accordance with the WI Nurse Practice.
- Participate in continuous quality improvement initiatives.
- Assume responsibility for professional development.
- Coordinate patient care and provide direct patient care.
- Conduct patient education.
- Perform telephone triage and eVisits.
- Assist with in-clinic minor procedures.
- Float to other areas as needed for operational needs.
- Perform other duties as assigned.
Required Skills
Core Skills
- Clinical experience in patient care and management.
- Strong communication and interpersonal skills.
- Ability to utilize evidence-based practice.
Preferred Skills
- At least 1 year of relevant nursing experience.
- Bachelor's Degree in Nursing.
Requirements
Experience: At least 1 year of relevant nursing experience preferred.
Education: Bachelor's Degree in Nursing preferred.
License & Certification:
- Current state of Wisconsin Registered Nurse License or Multi-state Nursing License from a participating state in the NLC.
- American Heart Association Basic Life Support for the Healthcare Provider (BLS) or equivalent required within 90 days of hire.
