Job Responsibilities
- Provide therapeutic patient and family centered care.
- Systematically and continuously collect and assess data related to patient health status.
- Develop, execute, and evaluate the plan of care.
- Utilize evidence to drive clinical decisions and practice related to individual and population health.
- Establish goals and strategies for meeting care coordination needs across settings.
- Delegate and supervise care in accordance with the WI Nurse Practice.
- Participate in continuous quality improvement initiatives.
- Assume responsibility for professional development.
- Coordinate patient care, provide direct patient care, and educate patients.
- Perform telephone triage/eVisits and assist with in-clinic minor procedures.
- Float to other areas as needed for operational needs.
- Perform other duties as assigned.

Requirements
Experience: At least 1 year of relevant nursing experience preferred.
Education: Bachelor's Degree in Nursing.
License & Certification:
- Current state of Wisconsin Registered Nurse License or Multi-state Nursing License from a participating state in the NLC
- AHA Basic Life Support for Healthcare Provider or equivalent required within 90 days of hire.
Schedule:
- Standard hours of 40.00 per week, with 2 Days WFH and 3 Days on-site.
