Job Responsibilities
- Ensure the delivery of high-quality clinical and home-based patient care.
- Promote optimal health outcomes and provide cost-effective solutions.
- Assess, develop, and coordinate care options tailored to the needs of members, typically 65 and older.
- Conduct home and virtual visits ranging from 45 to 60 minutes.

Requirements
Experience: Preferred: One (1) year of prior clinical or home care experience.
Education: Master’s degree from an accredited NP Program or PA program

- Active state Nurse Practitioner or Physician Assistant license
- Active RN License and Furnishing number (Must, upon hire)
- NP only Active Nurse Practitioner Board Certification/Physician Assistant Certification

- Valid BLS
- Valid driver license and current automobile insurance
