Job Responsibilities
- Assesses patients within scope of practice at every home visit.
- Carries out treatments per MD orders.
- Reports off to RN case manager and/or clinical manager.
- Reports all changes in condition to MD.
- Provides patient/caregiver education as per care plan.
- Completes all documentation within 24 hours.
- Includes changes in condition, follow-up with provider or other disciplines with supporting documentation.
- Attends all staff meetings, inservices, and all other meetings as required.
- Compliance with all mandatory requirements - inclusive of Healthstreams.
- Follows policies and procedures.
Required Skills
Core Skills
- Excellent verbal and written skills.
- Excellent clinical knowledge.
- Strong computer skills.
Preferred Skills
- Behavioral Health homecare experience.
Requirements
Experience: Minimum one year experience in the acute care setting. Home care experience preferred, but not required.
Education: Graduated from an approved Practical Nurses Education Program
License & Certification:
- Licensed Practical Nurse with Valid Connecticut license required.
- BLS certified.

- Per Diem

- Rotating Weekend and Holiday requirement
