Job Responsibilities
- Develop a specific behavioral health plan of care based upon comprehensive assessment, patient input and consultation with physician.
- Incorporate patient’s language, cultural and psychosocial needs into the care plan.
- Coordinate referrals for appropriate services/disciplines/DME/Supplies based upon skilled nursing assessment and physician’s orders.
- Assure timely and appropriate reassessment or discharge as per agency requirements.
- Actively participate in the REACT medication reconciliation program.
- Provide patient and family education for disease management and health promotion as directed by physician’s plan of treatment.
- Provide care to the utmost standard.
Required Skills

- New York State RN license
- Basic Life Support Certification
- Valid New York State Driver’s License
- Reliable transportation
Preferred Skills
- Minimum of one year of recent hands-on experience as a Registered Nurse
